What to bring to your WIC appointment

Bring the right papers and your first visit goes smoothly. Show up missing one, and you may have to come back. Here is what to have ready. Your clinic can confirm anything you are unsure about.

Who to bring

Bring every person applying for WIC, including your baby and children. Pack an extra clean diaper, since infants and children are weighed in a dry diaper.

Bring four things

1. Proof of who you are (ID)

Bring ID for yourself and each person applying.

  • For adults: driver’s license or state ID, passport or immigration card, birth certificate, Medicaid or insurance card, or voter registration card.
  • For infants and children: birth certificate or birth record, hospital ID bracelet, Medicaid card, medical record, or immunization records.

2. Proof of where you live

One paper with your name and street address (no P.O. boxes):

  • A current utility bill (power, water, gas, phone)
  • A current rent or mortgage receipt, or a pay stub showing your address
  • A valid Medicaid card or voter registration card
  • Mail addressed to you at your home

3. Proof of income

The shortcut: if you get SNAP, Medicaid, or TANF, just bring proof of that (your card or an eligibility letter). That covers the income step. Foster children are also covered.

If you do not have those, bring one showing the last 30 days: recent pay stubs, a Social Security or unemployment letter, child support records, or last year’s tax return.

4. A little patience for the first visit

Your first appointment is a longer certification visit. Staff check that you qualify, do a short health and nutrition screening, and set up your benefits. They will also show you how to use your WIC card at the store.

Can’t make it to the appointment?

You can name a proxy, someone you trust who can go to appointments and even shop for your WIC foods when you cannot. Ask your clinic for the proxy form.
